The death certificate was signed and submitted to a London registry by Osbourne’s daughter, Aimée Rose Osbourne. It revealed that he suffered a heart attack “out of hospital”.

He was then seen by an air ambulance and taken to Harefield Hospital in Uxbridge, London. Sadly, he passed away on July 22.

“It is with more sadness than mere words can convey that we have to report that our beloved Ozzy Osbourne has passed away this morning,” the statement read.

“He was with his family and surrounded by love. We ask everyone to respect our family’s privacy at this time.”

Just over two weeks before Osbourne’s death, he performed a concert in his hometown of Birmingham to a crowd of exhilarated Black Sabbath fans. Due to his ailing health, his family said the concert was his last performance ever.

“I’ve been laid up for six years,” he said to the crowd in the middle of the performance.

“Thank you from the bottom of my heart.”

Osbourne’s family honoured the late rocker publicly with a funeral procession through the streets in Birmingham last week. Thousands of fans lined the streets to pay their respects before the family held a private funeral.

Osbourne, who was nicknamed the Prince of Darkness for his eccentric onstage actions and gothic persona, is credited as one of the great minds behind heavy metal music, influencing a whole generation of musicians.

He also found a level of fame with a different crowd when he starred in the MTV reality show The Osbournes alongside his wife Sharon, and children, Kelly and Jack.
